Terms of Use for PulsePoint Medical Center Patient Portal

Introduction

Welcome to the patient portal ("Portal") of PulsePoint Medical Center ("we", "our", "us"). These Terms of Use govern your access to and use of the Portal. By using the Portal, you agree to these Terms of Use. If you do not agree with these terms, please do not use the Portal.

We may update these Terms of Use from time to time, and the updated version will be posted on the Portal. Your continued use of the Portal after any such changes signifies your acceptance of the updated terms.

 

1. Eligibility and Account Registration

To use the Portal, you need to register for an account and will become a patient of PulsePoint Medical Center. By registering, you confirm that:

-You are the individual patient, or the legal guardian of the patient, requesting access to the Portal.

-You are at least 18 years of age.

-The information you provide during the registration process is accurate and complete.

You are responsible for maintaining the confidentiality of your account credentials (username and password) and for all activities under your account. If you suspect any unauthorized access to your account, you must immediately notify us.

 

2. Access to the Portal

The Portal provides you with access to certain health-related information and services, including but not limited to:

-Viewing your health records, including test results and immunization history

-Booking, rescheduling, or cancelling appointments

-Communications and updates from PulsePoint Medical Center

We reserve the right to suspend or terminate your access to the Portal at our discretion, especially if there is any violation of these Terms of Use.

 

3. Patient Responsibilities

You agree to:

-Use the Portal only for lawful and personal purposes in accordance with these Terms of Use.

-Not use the Portal for any unauthorized, illegal, or harmful activities.

-Maintain the accuracy of the information you submit through the Portal and notify us of any changes to your personal information.

-Be responsible for any actions taken under your account, including communicating with your healthcare providers through the Portal.

-Review the information provided through the Portal regularly and contact us if you have any concerns or questions.

 

4. Privacy and Security

We are committed to protecting your personal health information and maintaining the privacy and security of your data. The Portal is designed to comply with the Personal Health Information Protection Act (PHIPA) and other applicable privacy laws in Ontario.

However, while we use security measures to protect your data, no system is completely secure. By using the Portal, you acknowledge and accept the inherent risks associated with electronic communication and storage of health information.

5. Limitations of the Portal

The Portal is intended as a convenient tool to access healthcare information and services. However, it is not intended to be a substitute for direct consultation with your healthcare provider. You should not rely on the Portal for emergency medical care or make healthcare decisions solely based on the information provided through the Portal.

If you experience a medical emergency, please seek immediate medical attention by calling 911 or visiting the nearest healthcare facility.

 

 

6. Intellectual Property

All content and materials on the Portal, including but not limited to text, images, videos, logos, and design, are the property of PulsePoint Medical Center or its licensors and are protected by copyright and other intellectual property laws.

You are granted a limited, non-exclusive, and non-transferable license to access and use the Portal for personal purposes in accordance with these Terms of Use. You may not reproduce, modify, distribute, or create derivative works based on the Portal without our prior written consent.

 

7. Disclaimers and Limitations of Liability

The Portal is provided "as is" without any warranties, express or implied, including but not limited to implied warranties of merchantability or fitness for a particular purpose. We make no guarantees about the accuracy, completeness, or timeliness of the information available through the Portal.

To the fullest extent permitted by law, PulsePoint Medical Center will not be liable for any damages, including but not limited to direct, indirect, incidental, special, or consequential damages arising from the use or inability to use the Portal.

 

8. Termination of Access

We may suspend or terminate your access to the Portal if you violate any of these Terms of Use or if we believe your account poses a security risk. You may also terminate your use of the Portal by notifying us.

Upon termination of your access, your right to access and use the Portal will end, but certain provisions (e.g., privacy, liability) will remain in effect.

 

9. Governing Law

These Terms of Use are governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein.
